With your permission, Kindness Initiative may share your information with qualified Service Providers. If needed, you will be asked to complete the “Authorization to Release/Exchange Information Form” at the link below; it is strictly up to you to decide whether you wish to to give your approval. You have the choice about the type of information wish to disclose, or how much information and for how long you authorize us to share information, and with whom we can share your information.
